What to bring on the first day?
Please bring your State Issued ID and Insurance Card.
What to expect on my First Day?
Patients must arrive at the clinic Monday through Friday between 5:00 AM and 8:00 AM in a state of withdrawal (ideally having not used substances within the past 24 hours).
The intake process will take approximately 3 to 4 hours and includes the following steps:

Evaluation by a Nurse, Doctor, and Counselor for a comprehensive biopsychosocial assessment

Collection of a urine drug screen and bloodwork
Please note: Medication on the first day is at the discretion of the doctor and will depend on the outcome of the full assessment.
If you are unable to complete the entire intake process during your visit, you will have the opportunity to return the next day to finish.
Will my take homes transfer with me?
All patients transferring from within the state of Kentucky will be required to complete a full intake assessment upon transfer.
At that time, we will adhere to all state and federal regulations regarding the continuation of take-home medications.
Decisions about take-homes will be made on a case-by-case basis, depending on:

Records received from the transferring clinic

The patient's current phase level
These details will be reviewed and discussed individually with each patient during the intake process.
